The Charles F. Bendzans Scholarship Fund gives students and artists from all backgrounds a chance to take Windsor Art Center classes at a discounted rate. If you need financial support and would like to apply for this scholarship, please email Michelle at classes@windsorartcenter.org

WINDSOR ART CENTER PHOTO RELEASE FORM

I hereby grant the Windsor Art Center and its successors permission to use, reproduce, and distribute my image and likeness in any photograph, film, video, or and other digital media taken at this Community Quilt Workshop, now known or hereafter developed, including web-based media, throughout the world in perpetuity, without payment or other consideration. I understand and agree that all photos of me taken at this Windsor Art Center event will be come the property of the Windsor Art Center and will not be returned. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo.

 

I HAVE READ AND UNDERSTAND THE ABOVE PHOTO RELEASE. I AFFIRM THAT I AM AT LEAST 18 YEARS OF AGE. I AM ALSO ACTING AS THE PARENT / GUARDIAN FOR THE ATTENDEES UNDER 18 YEARS OF AGE AND I AM GIVING CONSENT ON THEIR BEHALF AS EVIDENCED BY MY CHECK BELOW.
